Drug Screening Services
At JT Health, we provide professional, FMCSA-compliant drug testing designed to keep your fleet on the road and your workplace safe. As a certified collection site, we offer Federal DOT 5-Panel testing for regulated drivers, as well as comprehensive 10-Panel Non-DOT screenings for private employers. Whether you require pre-employment, random, or post-accident testing, we prioritize speed and accuracy to minimize downtime. With our direct partnership with MedTox/Labcorp and an independent Medical Review Officer, we ensure every sample is handled with the highest level of integrity and clinical oversight with all specimens tracked with chain of custody for all parties.
What to Bring for Your Test
Valid Driver’s License (or other government-issued photo ID).
Complete List of Medications: Include dosages and the names/contact info of the prescribing physicians.
Glasses or Contacts: If you require them for driving, you must wear or bring them for the vision test.
Hearing Aids: If you use them, ensure they are functioning and bring extra batteries.
Avoid Caffeine: For the most accurate blood pressure reading, avoid caffeine and nicotine for at least 2 hours before your appointment.
Requirements for Specific Conditions
Diabetes (Insulin-Treated):
Form MCSA-5870: You must bring a completed Insulin-Treated Diabetes Mellitus Assessment Form signed by your treating physician. This form must be dated within 45 days of your DOT exam.
Blood Sugar Logs: Bring 3 months of glucose records from your electronic meter.
Recent Lab Results: Bring your most recent Hemoglobin A1C test result (within the last 6 months).
Sleep Apnea (CPAP Users):
Compliance Report: Bring a printed "Usage Report" from your CPAP machine covering the last 90 days.
The Standard: You must demonstrate at least 70% usage (minimum 4 hours per night on 70% of nights).
Heart Conditions (Stents, Pacemaker, Bypass, etc.):
Cardiologist Clearance: A letter from your cardiologist stating you are safe to operate a commercial vehicle from a cardiac standpoint.
Recent Testing: Bring a copy of your most recent Stress Test or Echocardiogram (usually within the last 1–2 years).
Pacemakers: Documentation of your most recent annual pacemaker check.
Seizures & Neurological Conditions:
Neurologist Clearance: A letter on letterhead stating your diagnosis, medication stability, and a statement of support for you driving a commercial vehicle.
Seizure Exemption: If you have an active FMCSA Seizure Exemption, you must bring a copy of the certificate.
